Le titre reprend le principe instauré par "Downtown Nekketsu March", une compétition sportive dans l'univers de Kunio-kun, donc tous les coups, que ce soit de poing, pied ou avec des armes contondantes, sont permis !
Des arrière-plans en 3D sont de mise tout en gardant les sprites 80's des personnages. Outre un mode histoire mettant en scène Kunio-kun et ses potes du lycée Nekketsu, il existe un mode bataille libre pour 4 joueurs hors ligne, ainsi qu'un mode en ligne dans lequel il est possible d'utiliser les courses du mode création.
